The CHAP (comprehensive Housing Assistance Program) Case Manager provides housing location or relocation support, budget counseling and case management to clients referred to the program. This position is a contracted position with Penn Medicine/Lancaster General Health.

Key Qualifications

  • Minimum Education Requirements- 2-year post-secondary education required. Bachelor Degree in Social work preferred
  • One year of relevant experience required
  • Ability to speak, read and write English and Spanish required
  • Excellent communication skills especially listening, mediation and writing skills
  • Strong organizational skills with ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance
  • Commitment to the Housing First Philosophy
  • Basic knowledge of tenant’s rights and responsibilities case management best practices
  • Concentrated record keeping, budgeting and mathematical skills
  • Demonstrated knowledge of county social service agencies and community programs
  • Experience with computer and proficiency with Microsoft Office suite
  • A commitment to empowering others to solve their own problems
  • A conviction about the capacity of people to grow and change
  • Establish trusting relationships with persons served to help them gain skills and confidence
  • The ability to work collaboratively with other personnel and/or service providers
  • Valid driver’s license and legal vehicle that is registered, inspected and insured
  • Willingness to travel throughout Lancaster city and county
